1 The employee must indicate on the injury report form whether or not medical treatment was received.  Injuries not requiring medical treatment are not reported to our workers’ compensation insurance carrier. The Employee and Supervisor Injury Report can be found on the ISD website under Human Resource General Forms.

2 It is imperative that these reports be forwarded WITHIN 24 HOURS OF INJURY.  Our workers compensation insurance company will deny payment of medical bills if an injury report is not on file.  Additionally, if further treatment or a referral is indicated, the insurance company cannot authorize such treatment without an injury report and claim number. 

3 Should the employee decide at a later date to seek medical treatment, he/she must call Janel Faber so that the injury report information can be forwarded to the insurance company.

WORKERS’ COMPENSATION – ADDITIONAL INFORMATION

Treatment for Injuries: 

Employees are urged to seek medical treatment as soon as possible after the injury.  All initial treatment must be handled through the ProMedica 360 Health which is located at 901 North Macomb St., Suite 1 in Monroe.  This is across the street from the main hospital campus.

Follow-up Procedures:

After being seen at ProMedica 360 Health, employees are expected to report to the Human Resource Department with the “Return to Work Recommendation”.  If the employee is given restrictions, the Assistant Superintendent for Human Resources will contact the supervisor to determine if the employee will be able to work with the stated restrictions. 

Note:  If an employee has restrictions, he/she must continue to follow-up with ProMedica 360 Health until the restrictions are lifted.  The “Return to Work Recommendation” must be brought to the Human Resources Department following each appointment and prior to reporting back to work.

Evening or Weekend Treatment

Should an employee need treatment for a work-related injury during hours that the ProMedica 360 Health is not in operation, employees are to be seen at the Emergency Department at ProMedica Monroe Regional Hospital.
